We’re here to make getting the right support simple
Choosing a disability support provider is an important decision. At Strive Together WA, we understand that every person has different goals, preferences, routines and support needs.
Our referral and onboarding process is designed to make joining Strive as straightforward as possible, while ensuring we have a clear understanding of the person we are supporting and the services they are looking for.
Whether you are an NDIS participant, family member, guardian, Support Coordinator, NDIS representative or another person involved in a participant’s support network, our team will work with you throughout the process.
We take a person-centred approach, meaning the individual is at the centre of the conversation. We want to understand what is important to them, what they would like to achieve, how they prefer to receive support and what they need from their support team.
Our onboarding process helps us make sure the right information is collected, the appropriate supports are identified and everything is in place before supports commence.
The Referral Process

The first step is to contact Strive Together WA to discuss the support you or the person you represent is looking for.
At this stage, we will talk with you about the type of support required and gather some initial information to help us understand whether our services may be suitable.
We may discuss things such as:
The participant's current support needs
Their goals and what they would like to achieve
The type of support they are looking for
Preferred days and times for support
Preferred locations
Current living arrangements, where relevant
Community, social or employment goals
Existing supports
NDIS funding and the relevant support categories
Where appropriate, we will also work with the participant's family, guardian or Support Coordinator to ensure everyone involved has a clear understanding of the referral.

Before supports can commence, we need to understand the participant's NDIS plan and the funding available for the requested services.
We will confirm relevant details of the participant's plan, including:
NDIS plan number
Plan start and end dates
The type of plan management arrangement
Relevant funding categories
Relevant support items
The supports being requested
The anticipated hours and frequency of support
This helps ensure that the proposed supports are appropriate and align with the participant's available NDIS funding.
For participants who are Agency-managed, Plan-managed or Self-managed, we will discuss the relevant arrangements during the onboarding process.
If you are a Support Coordinator, Plan Manager, guardian or family member assisting with the referral, we may request relevant plan and funding information to help us establish the appropriate support arrangements.

Once the referral has progressed, we will arrange an onboarding meeting with the participant and, where appropriate, their family member, guardian and/or Support Coordinator.
This meeting is an important part of the process. It gives us the opportunity to get to know the person beyond their NDIS plan and understand what matters to them.
We will talk about the participant's:
Goals
What are they working towards?
What would they like to achieve through their supports?
Preferences
How do they like to communicate?
What activities do they enjoy?
Are there particular routines, environments or approaches that work best for them?
Support Needs
What assistance do they require throughout their day?
What would they like support staff to help them with?
Expectations
What does a good support service look like to them?
What are their expectations of their support team?
Our aim is to ensure the support we provide is not simply based on a list of tasks, but is tailored around the individual and what is important to them.

During the onboarding meeting, we will discuss the areas where support may be required.
Depending on the individual, this may include:
Organisation and planning
Time management and punctuality
Developing independence
Daily living activities
Household tasks
Community access
Social participation
Developing social connections
Employment assistance
Support through changes or transitions
Participation in Connect Groups
Medication support and administration
Other individual support requirements
The purpose of this conversation is to build a clear picture of the support the participant requires and how Strive Together WA can best provide it.
We encourage participants, families and support professionals to share information that will help us understand the person's needs, preferences, routines and communication style.

As part of onboarding, we will work through the information and documentation required to establish the participant's support arrangements.
This may include information relating to:
Personal details
Emergency contacts
Medical information
Allergies
Communication preferences
NDIS plan details
Consent
Support requirements
Risk and safety considerations
Service arrangements
We will provide the required onboarding documentation and explain what is needed.
Where appropriate, documents may be completed electronically.
We understand that there can be a lot of information involved when starting with a new provider, so our team will guide you through what is required.

Before supports commence, the required consent documentation needs to be completed.
This allows Strive Together WA to communicate appropriately with the people involved in the participant's support and manage information in accordance with our requirements.
Consent may cover areas such as:
NDIS-related communication
General consent
Media consent, where applicable
We take the privacy and confidentiality of participant information seriously. Information provided to Strive Together WA is managed securely and in accordance with our privacy and record management requirements.

The information gathered throughout the referral and onboarding process is used to develop an understanding of the participant's support requirements.
Your support plan helps ensure that the team understands:
What support the participant requires
What is important to the participant
Their goals and preferences
How they communicate
Relevant routines
Areas where they would like to build independence
Any important medical or safety information
The type and frequency of support required
The support plan provides an important foundation for delivering consistent, person-centred support.

Before supports begin, we will confirm the proposed service arrangements.
This includes discussing:
The type of support being provided
Preferred days and times
Locations
Expected hours of support
Relevant NDIS funding
Communication arrangements
Cancellation requirements
Reporting requirements
Other relevant service expectations
We will also ensure that the proposed support arrangements align with the participant's available NDIS funding.
Where a service agreement is required, this will form part of the process of confirming the participant's support arrangements with Strive Together WA.

Before the first support takes place, our team completes the necessary checks to make sure the participant is ready to commence.
We confirm that:
Required documentation has been completed
Necessary consents have been provided
Relevant information has been recorded
Support requirements have been identified
Proposed supports align with available funding
The participant's support arrangements have been confirmed
The relevant team members have the information they need to provide appropriate support
Once everything has been approved and arrangements are in place, your first support can be scheduled.

What happens if funding or support requirements change?
NDIS plans and individual circumstances can change over time.
As a participant approaches the end of their plan, we work with the participant and relevant members of their support network to understand whether there are upcoming changes to their plan or funding that may affect their supports. This may include communicating with a Support Coordinator or other authorised person to understand the status of a plan review and any expected changes to funding.
We encourage participants and their support networks to keep us informed of any changes that may affect their services so that we can work together to plan appropriately.
How long does the referral process take?
Every referral is different, and the time required to complete the process will depend on the individual's circumstances, the type of support being requested, the information required and the arrangements that need to be put in place.
The more quickly we can receive the required information and documentation, the more quickly our team can work with you to progress and process your referral.
Providing clear and complete information at the beginning of the referral process helps us better understand the participant's needs, confirm the appropriate supports and funding, and move through the onboarding process as efficiently as possible. We understand that gathering information can take time, and our team is here to help guide you through what is required. If information is missing or further clarification is needed, we will work with you, the participant, family members, Support Coordinator or other relevant representatives to obtain what is needed.
Our priority is to ensure that the referral is completed thoroughly and that the appropriate arrangements are in place before supports commence, while working with you to progress the process as efficiently as possible.
